2、place of issuance 護照的簽發地點,city 城市,country 國家（國內的,選擇china-mainland）,state/province 省份（直轄市可以不填）

3、issuring country 護照簽發國家（國內的,選擇china-mainland）

4、issuance date (dd-month-yyyy) 護照簽發時間,這一項與普通ds156表格有一點區別,普通的156表格的日期格式為mmm,而網上的156,直接選擇就可以了。

5、expiration date (dd-month-yyyy) 護照過期時間（填寫方法同上）

6、surnames (as in passport) 護照上的姓（拼音）

7、first and middle names (as in passport)護照上的名（拼音）

8、other surnames used (maiden, religious, professional, aliases) 其他姓

9、other first and middle names used 其他名

10、date of birth (dd-month-yyyy) 出生日期（“月份”那一項,直接選擇就可以了）

11、place of birth 出生地點

12、nationality 國籍（選擇china-mainland就可以了）

13、sex 性別（直接選）

14、national identification number 身份證號碼

15、home address (include apartment number, street, city, state or province, postal zone and country) 家庭地址（包括房間號碼,街道,城市,省份,郵編和國家）

16、電話號碼（包括家庭電話、辦公電話、移動電話、傳真號碼、工作傳真號碼、尋呼機pager number,有幾個填幾個,沒有就不填）

17、marital status 婚姻狀況（married——已婚, single (never married)——單身（從未結婚）, widowed——寡居, divorced——離婚, separated——分居）

18、 spouses full name (even if divorced or separated. include maiden name.) 配偶的全名（即使離婚或分居了也要填,包括婚前姓）——外國女人結婚后,可能把自己的姓氏改為丈夫的姓氏,所以這里才提出“婚前姓”,中國人不用考慮這一 點。

19、spouses dob (dd-month-yyyy) 配偶的出身日期（dob是date of birth的縮寫）

20、name and address of present employer or school 當前學校或者雇主的名字和地址（應屆生填寫學校地址即可。若是畢業一段時間了, 而且又沒有找到工作,可以不填）

21、present occupation (if retired, write "retired". if student, write "student".) 當前的職業（如果退休了,填“retired", 如果是學生的話,填"student")

22、 when do you intend to arrive in the us? (provide specific date if known) 打算什么時候到美國？（如果知道具體日期的話,請注明具體日期） 填寫i20上的報到時間,好像也可以提前兩三天。

23、e-mail address 電子郵件地址

24、at what address will you be staying in the u.s.?在美國的居住地點 （填寫i20上的學校地址即可）

25、 name and telephone numbers of person in u.s. who you will be staying with or visiting for tourism or business 邀請人的名字,你去拜訪的人是誰（對于學生來說,name填學校的名稱,電話填國際學生辦公室或者申請的院系的電話）

26、how long do you intend to stay in the u.s.? 你打算在美國停留多久？（i20上的學習時間）

27、what is the purpose of your trip? 此行的目的（f1就填study)

28、who will pay for your trip? 誰付錢？（對于學生來說,自費的可以寫funds from family (或者是parents),獲得獎學金的可以寫funds from xxx university）

29、have you ever been in the u.s.? 曾經去過美國嗎？

30、have you ever been issued a u.s. visa? 曾經獲得過美國簽證嗎？

31、have you ever been refused a u.s. visa? 曾經被拒簽過嗎？

32、do you intend to work in the u.s.? 有沒有在美國工作的意向？（學生肯定要填no）

33、do you intend to study in the u.s.? 有沒有在美國學習的意向？

34、names and relationships of persons traveling with you 隨行人員的名字,以及和你之間的關系（除攜行外,不要填寫。或者填寫none）

35、has your u.s. visa ever been cancelled or revoked? 您的美國簽證是否被吊銷過？（填no）

36、has anyone ever filed an immigrant visa petition on your behalf? 是否有人為您申請過美國移民簽證？（填no）

37、 are any of the following persons in the u.s. or do they have u.s. legal permanent residence or u.s. citizenship? mark yes or no and indicate that persons status in the u.s. (i.e., u.s. legal permanent resident, u.s. citizen, visiting, studying, working, etc.). 下列親屬中是否有人已在美國？他們是否是美國合法永久居民或美國公民？請注明“在美國”或“不在美國”。如選擇“在美國”,請注明其在美國的身份(即是美 國合法永久居民還是美國公民,是否正在美國訪問、留學、工作等)。

（1）、只在與自己有關的家庭成員前面選擇。無關的,比如無兄弟姐妹,就不用選擇

（2）、非直系親屬不用填,比如表姐妹、表兄弟。

填表之后,點擊網頁下方的“continue",等一會兒,就會自動生成一個ds156表（共三頁）。檢查無誤、打印出來以后,在頁貼上照片,第二頁簽名就可以了。簽證的時候帶齊3頁
